Better stroke care, everywhere: Study boosts local hospitals' clotbuster use

Dec. 20, 2012 ? From the moment a stroke occurs, patients must race against the clock to get treatment that can prevent lasting damage. Now, a new study shows the promise -- and the challenges -- of getting them state-of-the-art treatment safely at their local hospital, saving precious minutes.

The results come from an effort that tested methods to improve delivery of a time-sensitive, clot-busting drug in stroke patients at 24 community hospitals across Michigan. To date, clot-busting treatment has been mostly used at larger hospitals.

The research effort was coordinated by members of the University of Michigan Health System's Department of Emergency Medicine, Department of Neurology and Stroke Program, which offered half the hospitals education and round-the-clock treatment assistance by phone. The study was funded by the National Institute of Neurological Disorders and Stroke at the National Institutes of Health.

By the end of the study, the community hospitals across Michigan that had the U-M experts as the "sixth man" on their teams did better at delivering the drug called tPA to eligible patients than those that didn't.

The findings of the randomized controlled trial are published in Lancet-Neurology. They show that community hospitals can indeed improve patients' chances of getting tPA in the first few hours of a stroke, without increased risk of dangerous bleeding.

Data from 22 of the hospitals show that tPA use more than doubled in the 11 hospitals that were randomly chosen to get the extra help, versus a smaller increase in the 11 that didn't. Some hospitals even surpassed national targets for tPA use that large stroke centers don't always reach -- a true game-changing performance.

Across the U.S., less than 2 percent of stroke patients receive tPA -- when more than 11 percent could -- largely because of the time limits on its use and delays in getting patients to a hospital. That's why it's important for community hospitals to offer it.

The investigation, called INSTINCT for INcreasing Stroke Treatment through INterventional Change Tactics, demonstrates that tPA can be used safely and appropriately in the community hospital setting -- and that more work needs to be done to expand public access to the only treatment approved by the U.S. Food and Drug Administration to reverse the effects of stroke. While improvement at the target community hospitals that got the education was statistically significant, it was not as large as hoped for. But the findings suggest that relatively low-cost and low-tech interventions can improve local stroke care.

Lead author Phillip Scott, M.D., a U-M emergency physician and principal investigator of the trial, likens the results to the performance of teams from smaller athletic conferences that beat larger, more celebrated teams, to reach the final stages of the annual NCAA college basketball tournament.

"This study suggests that community hospitals can evaluate and treat enough emergency stroke patients to keep their teams' treatment skills sharp, but are of a size that creates rapid lines of communication and stable physican and nurse teams. This may facilitate rapid, safe stroke diagnosis and care," says Scott, an associate professor of emergency medicine at the U-M Medical School. "Four of the hospitals that received the educational intervention achieved tPA use rates of 5 to 8 percent of all stroke patients, compared to only one of the control hospitals. That's a rate many large medical centers strive to reach."

"This study, while finding only modest improvements, provides encouraging evidence that intensive professional education at community hospitals has the potential to improve the use of tPA in acute stroke care," said Scott Janis, Ph.D., program director at the NINDS. "Importantly, it also lays the groundwork for future strategies that should be explored to develop evidence-based interventions that would improve patient access to this proven therapy following stroke."

How it was done, and more about the results:

The study started by collecting baseline data on stroke treatment from 2005 and 2006 at all the hospitals. After the sites were randomized, the U-M team offered continuing medical education classes at the hospitals, workshops at U-M, and other support, including 24-hour phone consultation availability, to the hospitals chosen to receive the intervention.

All the hospitals were in the lower peninsula of Michigan, and were chosen at random from among hospitals with at least 100 stroke discharges per year. All hospitals that participated stayed in the trial until data collection ended in 2010. Hospitals could hold primary stroke center designation, as granted by the Joint Commission, but could not be tertiary academic comprehensive stroke centers.

In all, 188 tPA treatments occurred among 15,065 stroke patients during the baseline period, and 557 treatments occurred among 25,758 stroke patients from 2007 to 2010. The authors note that tPA use at hospitals nationwide rose somewhat during the study period, due to factors such as insurance reimbursement and new data on tPA's effectiveness at preventing disability.

The INSTINCT study findings were complicated by the fact that one of the hospitals enrolled in the trial became an academic stroke center after starting the study. When it and its matched hospital are excluded from the analysis, the remaining hospitals showed enough of an impact from the educational effort to be statistically significant -- a 105 percent increased use of tPA over baseline.

"We essentially saw a doubling of tPA usage, using standard technologies for education and support. Importantly, the increase was achieved safely," Scott says. "This shows we can translate the knowledge of effective stroke treatment into a community setting." The size of the study, with 557 stroke patients treated after intervention began, makes it one of the largest tPA studies in the world.

The use of tPA in ischemic stroke has grown around the world in the last decade, but many patients who could get the drug still do not. The "treatment window" for tPA has been seen as three hours from the onset of stroke symptoms, though recent studies suggest patients can see benefit even four and a half hours out. The importance of early treatment, however, cannot be overstated, as patients treated earlier have greater benefits and the drug is not currently FDA-approved for use beyond three hours.

Considering that most stroke sufferers wait more than an hour after symptoms start before they seek help, and that travel time to a tPA-providing hospital is around 30 minutes, that leaves hospitals about an hour to do medical imaging, make a firm diagnosis and initiate tPA treatment.

Patients must then be monitored to check for brain bleeding, and to assess the impact of their stroke. To reliably accomplish this, a system for the care of acute stroke patients must already be in place when a patient arrives at the emergency department.

Patients who don't reach a location where they can received standard tPA treatment in time can still potentially be treated with advanced catheter-based clot removal treatment at a major referral hospital such as U-M. These advanced therapies have a longer time window for treatment, but have not been proven more effective than early standard treatment. The Stroke Program at U-M is participating in multiple trials to improve advanced stroke treatment.

Does agriculture have a future? Sonali Bisht wraps up ... - Oxfam Blogs

For the past two weeks, Oxfam has been hosting an online forum on the future of agriculture with a great range of viewpoints fromsonya-bishit-220x220 every corner of the globe. Today is the last blogging day before the Christmas break (see you in 2013, everyone), so I?m handing over to Sonali Bisht, founder of INHERE, India, to wrap up a pretty diverse set of discussions.

The technology to produce synthetic food exists. Food pills are only one step beyond the vitamins, proteins and other food and nutrient supplements currently available in the market. We have knowledge of hydroponics and we can grow food in multi-storey production complexes. Certainly, there are plenty of alternatives to traditional farming for food and other needs. Does agriculture, as we know it have a future?

The experts who contributed to the Future of Agriculture debate, all eminent persons, leaders in their field, chose not to address such radical alternatives, and the comments received did not dispute that choice. Clearly, food grown by people living in rural areas, especially smallholders, is seen as important for the future.

?The technology to produce synthetic food exists.?

Smallholders currently constitute the majority of agricultural producers, the bulk of the poor and half the world?s hungry. They are expected to continue producing for a growing and more affluent urban population, and to do so in ways that keep food prices low, preserve the environment and manage the multifaceted risks they face, including vulnerability to shocks from the natural, socio-economic and political environment. The risks and vulnerabilities faced by women and indigenous populations, and expected to be managed by them, are even greater.

The experts generally offer optimistic visions for the future of agriculture, though the reasons for their optimism vary. Experts with a background in agriculture research and industry put their faith in fossil-fuel and chemical-based agriculture to achieve the increases in productivity needed to feed the population of the future. Or they champion comparative advantage, open trade and functioning markets.

Experts with a civil society background, on the other hand, believe high production levels can be obtained without chemical- or fossil-fuel-based inputs. They cite evidence that organic and sustainable agriculture achieves equivalent production in normal years and higher in drought or abnormal years. They also see sustainable and organic agriculture as empowering for women farmers, valuing their role and knowledge in agriculture, and helping to keep them and their families out of crippling debt. And several view food sovereignty as more important than markets.

The primacy of smallholders was acknowledged by almost all the experts. Several maintained that smallholders can generate research knowledge and use it for their prosperity, noting that peasants already make an enormous contribution in that regard.

If farming is to continue, youth need to pursue it as a career. But at present, farming is not an occupation young people aspire to and smallholder farming is not perceived to be a respected occupation. Agriculture is not given the status of a skilled craft in most countries, and thus wages of unskilled labour apply. This situation can and must change in developing and developed countries alike.

Farms need to be managed as profitable businesses if they are to attract a new generation of farmers. Perhaps, as Nicko Debenham suggests, some form of community or group enterprise would offer a sustainable business model that could generate a ?more-than-acceptable living.? I wonder if that would appeal to Susan Godwin, who wants secure land tenure and more access to information for her daughter. Or to Rokeya Kabir, who says women farmers deserve more for the hard work they put in.

The views expressed were many and too rarely did those of opposing views engage each other. Pro- and anti- food sovereignty views were

left unresolved. Much of the debate resided in the realm of hope, perhaps best expressed by John Ambler, who envisaged institutional reforms leading to healthier eating and a healthier food system.

?The reality is that it has been difficult to build political will that favours smallholders.?

The underlying challenge has always been politics. As Prem Bindraban observed, power structures, vested interests, economics and other

drivers influence decisions in agriculture. Participants in the debate voiced this sentiment in different ways to express skepticism as well as hope. But the reality is it has been difficult to build political will that favours smallholders.

There is an Indian saying that the one who is thirsty goes to the well. The well does not come to him. Yet, without exception the experts feel farmers should produce for the market, conduct market intelligence, take their produce to the market.

One would think that if food is a priority need of consumers the initiative would come from them or their representatives. The consumer, who is generally urban and has higher income, should take responsibility for creating reserves to account for the vagaries of weather and for insurance against price fluctuations. The farmer should be in the position to decide whether he or she can produce at the price consumers offer or if further negotiations are needed. Community-supported agriculture, where communities invest in farmers by subscription, is a model that is worth more attention, as it guarantees farmers a fair price and assures consumers of clean and safe food, while sharing the risk.

Mostly this does not happen. Politicians have their constituencies to please, and most of these are non-farmers living in wealthier areas of the country. Private companies view agriculture as an unending stream of business and profits. The political power of the fossil-fuel industry and the lobbying clout of agribusiness keep agriculture dependent on fossil fuels.

Nonprofit NGOs, though always strapped for resources, can create models of excellence which demonstrate the success of innovations. But these are rarely replicated at scale. Research institutions create knowledge which the poor are unable to access and use, while private companies can and do, often at a fraction of the real cost.

The consumer, especially the urban consumer, tends to be king in agriculture. Companies vie for a percentage of his or her essential spending and governments pander to the needs of this majority. Good intentions tend to get lost in this realpolitik. Until aware consumers change their behaviour, the smallholder farmer will get good words, symbolic gestures, and little else.

?Farmers need to be recognized as co-creators of knowledge in agriculture, encouraged and respected for the innovations they develop.?

It would not cost very much to make changes that, by common consensus, would transform the future of agriculture for rural poorAgribusiness (potato) - naugon Akhoria_opt people. Farmers, especially women, need security of land tenure or land ownership and protection against land grabbing. Farmers require fair prices for their produce and ways of farming which do not get them into debt and food insecurity.

Above all, most experts and respondents agree, farmers need to be recognized as co-creators of knowledge in agriculture, encouraged and respected for the innovations they develop. Farmers and research institutions must be linked in a web of knowledge creation and application, with joint responsibility for improving production and productivity through joint trials, participatory innovation, and farmer validation of scientists? claims. This is the key to meeting production challenges in the agriculture of the future.

?Agriculture not only feeds people, it, builds close-knit families and societies.?

National systems and multilateral agencies should support this process with NGOs and farmers? organizations facilitating accountability.Indian farmers Planning of production for local markets and according to local needs would avoid mismatch and waste. Application of force majeure clauses in production agreements would eliminate much of the risk. Subsidies and artificially lowered prices of commodities as social welfare measures have proven to be hotbeds of corruption and disincentives to farmers and should be avoided.

Agriculture not only feeds people, it creates engagement and employment in sustainable livelihoods, builds close-knit families and societies (especially smallholder and family farming) and supports cultural and social engagement as well as social stability. In today?s world it provides an alternate way of living from the stress and strain of urban areas. It preserves our farm landscape, traditions and heritage. We all have a responsibility to preserve and enhance our agricultural heritage?and that means not allowing a single farmer or farm labourer to go hungry or to suffer for being involved in agriculture.

New study sheds light on dinosaur size

Dec. 19, 2012 ? Dinosaurs were not only the largest animals to roam the Earth -- they also had a greater number of larger species compared to all other back-boned animals -- scientists suggest in a new paper published in the journal PLOS ONE.

The researchers, from Queen Mary, University of London, compared the size of the femur bone of 329 different dinosaur species from fossil records. The length and weight of the femur bone is a recognised method in palaeontology for estimating a dinosaur's body mass.

They found that dinosaurs follow the opposite pattern of body size distribution as seen in other vertebrate species. For example, within living mammals there tends to be few larger species, such as elephants, compared to smaller animals, such as mice, which have many species. The evidence from fossil records implies that in contrast there were many species of larger dinosaurs and few small species.

Dr David Hone from Queen Mary's School of Biological and Chemical Sciences, explains: "What is remarkable is that this tendency to have more species at a bigger size seemed to evolve quite early on in dinosaurian evolution around the Late Triassic period, 225 million years ago, raising questions about why they got to be so big.

"Our evidence supports the hypothesis that young dinosaurs occupied a different ecological niche to their parents so they weren't in competition for the same sources of food as they ate smaller plants or preyed on smaller size animals. In fact, we see modern crocodiles following this pattern -- baby crocodiles start by feeding off insects and tadpoles before graduating onto fish and then larger mammals."

Dr Eoin Gorman, also from Queen Mary's School of Biological and Chemical Sciences added: "There is growing evidence that dinosaurs produced a large number of offspring, which were immediately vulnerable to predation due to their smaller size. It was beneficial for the herbivores to grow to large size as rapidly as possible to escape this threat, but the carnivores had sufficient resources to live optimally at smaller sizes.

"These differences are reflected in our analyses and also offer an explanation why other groups do not follow a similar pattern. Several modern-day vertebrate groups are almost entirely carnivorous, while many of the herbivores are warm-blooded, which limits their size."

Is Social Media really so far behind in the mindset of executives, especially in B2B? Well, according to the Zeno?s Digital Readiness Survey conducted by Harris Interactive it is. The poll asked 300 U.S. corporate executives of various industries and titles of VP or higher, including?C-suite executives (primarily B2B) with annual revenues of at least $1 billion. The study comes to a conclusion that surprises us: Many executives fail to consider Social Media reputation when making business decisions. Over one-third of executives (36%) stated that the CEO of their company does not care or cares little about the company?s reputation in Social Media.

Although many companies out there like us advice the leading management how to work with Social Media and how to turn the company into a Social Business, the findings show that still 10% of organizations do not take any action at all to engage with audiences online to address a damaging article or Social Media post. And when it claims that managers would at least take some action to respond to an online crisis, it tells me that Social Media is still not a hotspot for companies and brands.

The main findings of the survey?
- B2B executives (43%) say their CEO largely ignores their company?s online reputation (B2C only 30%) when making business-decisions.
- B2B executives are slower in response. Only 45% of business executives see their company can respond to a negative online post within 24 hours (B2C 63%)
- B2B executives are twice as likely (13%) to say that their firm would not engage an audience online at all to defend their reputation (B2C 6%)
- Executives in larger firms (10,000 employees+) are more likely to say their CEO always or sometimes considers their company?s Social Media reputation versus those in smaller companies (71% versus 55%). ?
- Executives in smaller firms ignore Social Media reputation when considered business decision-making more than larger firms (45% versus 29%)

?Given the explosive growth of today?s digital platforms, the Zeno Digital Readiness Survey shows a much larger percentage of companies than one would expect turning a blind eye to valuable customer views and insights. (?)?These businesses, regardless of sector, risk serious reputational damage, as well as miss out on important stakeholder feedback, when they ignore social media conversations about their companies and their industries.? AUGUSTA, G.A.--- News 12 has learned a pedestrian has been hit and killed tonight.

Deputy Coroner Johnny McDonald says the victim is Rapha Ray Felts, 48, from Washington, North Carolina.

He says Felts was in the area on a construction job.

It happend around 8:30, Tuesday night. The man was hit by a Chevorlet truck traveling eastbond on Washington Rd. Richmond Co.

Sgt. Danny Whitehead said the driver was not arrested and says "there will be no charges at this time."

Deputy Coroner McDonald says Felts died of blunt force trauma to the head and body.

This is the second person who has been hit and killed in the past 24 hours in Augusta.

4 EU nations to condemn latest Israel settlements

PARIS (AP) ? Germany and the three other West European members of the U.N. Security Council are preparing a statement condemning Israel's latest settlement plans in east Jerusalem and the West Bank, two European diplomatic officials said Tuesday.

Permanent council members Britain and France plus Germany and Portugal ? who are wrapping up two-year terms ? are concerned that such settlements could threaten a possible two-state solution between Israel and the Palestinians, the officials said. Details of the statement are being finalized in New York, the diplomats said on condition of anonymity because the work was not yet completed.

The two officials said the statement of condemnation will be presented to council members, but the European countries are not seeking to have it approved as an official statement by the council or as a resolution ? most likely because of near-certain opposition by the United States, Israel's closest ally.

The European statement is likely to be issued on Wednesday, said a U.N. diplomat in New York, speaking on condition of anonymity because negotiations have been private.

The move stems from a string of recent Israeli settlement announcements, the diplomats said, including one on Monday in which Israel said it will push forward with plans to build 1,500 apartments in east Jerusalem, the Palestinians' hoped-for capital.

The statement by key European countries on the U.N.'s most powerful body would be a symbolic, but nonetheless high-profile show of displeasure with the Israelis.

The U.S. government on Tuesday also stepped up its criticism, delivering a rare blunt rebuke to its top Mideast ally for its new settlement construction. State Department spokeswoman Victoria Nuland accused Israel of engaging in a "pattern of provocative action" that runs counter to the government's commitment to peace.

British Foreign Secretary William Hague on Tuesday called all Israeli settlements "illegal under international law." He urged Israel to reverse its latest expansion plan in east Jerusalem, warning that if implemented "it would make a negotiated two-state solution, with Jerusalem as a shared capital, very difficult to achieve."

The European Union, Israel's biggest trading partner, has been increasingly vocal in its criticism of new settlements just as Israel is gearing up for general elections next month. In an unprecedented move, a string of European governments summoned their local Israeli ambassadors to lodge protests following the Israeli settlement announcements.

Israeli Prime Minister Benjamin Netanyahu has been unshaken by the criticism, and on Tuesday he vowed to continue building in east Jerusalem. "Jerusalem is the eternal capital of the state of Israel, and we will continue to build there. A united Jerusalem expresses a wide national agreement," he said in the northern Israeli town of Acre.

Israeli Foreign Ministry spokesman Yigal Palmor declined to comment on the reported European initiative in the Security Council, since nothing formal has been proposed. But Palmor said the idea of taking action at the United Nations only lowers the chances of renewing peace talks, and he insisted the only way to advance negotiations is "to weigh on the Palestinians and convince them to return to the negotiating table."

"Fiddling with U.N. resolutions will take us the opposite way," he added. "So it's their choice to make, a step forward or two steps backward."

One of the European diplomats said the proposed statement of condemnation would be a "political message" distributed to all the other council members but that the Europeans are not asking the council to adopt a formal "presidential statement" or less important "press statement." Each of those would require approval by all council members.

In such cases, the United States has often stepped in on the side of Israel on the recurrent issue of settlements.

Almost exactly a year ago, the same four European nations issued a statement critical of Israeli settlements at the U.N. Security Council. They and the other 10 members pointed a finger at the United States for blocking any condemnation of Israel's accelerated settlement construction.

The United States vetoed a U.N. resolution in February 2011 that would have condemned "illegal" Israeli settlements and urged an immediate halt to all settlement building. The 14 other Security Council members voted in favor of the resolution.

German Chancellor Angela Merkel has repeatedly spoken out against the settlements, but her government is wary of being too tough on Israel ? whose safety she has declared to be a key German interest. Germany is considered one of Israel's staunchest political allies in the European Union, as skepticism about Israel's policies is growing across the 27-nation bloc.

When Netanyahu visited Berlin earlier this month, Merkel said their differing views of Israel's settlement policies were well known, but she went to great length to stress the solid bond between the two countries which trumps those differences.

Netanyahu announced plans to build thousands of homes in settlements in the West Bank and east Jerusalem in response to the U.N. General Assembly's decision last month to upgrade the Palestinians' status to nonmember observer state.

Israel's closest European allies ? Britain, Germany, Italy and France ? all abstained or voted with the Palestinians. The Assembly decision recognized a Palestinian state in the West Bank, east Jerusalem and Gaza Strip, territories captured by Israel in the 1967 Mideast war ? an annexation that has not been recognized internationally. Israel rejects a return to the 1967 lines and accused the Palestinians of bypassing negotiations with the U.N. bid.

Peace talks have been frozen for four years, in large part because of the settlement issue. The Palestinians refuse to negotiate while Israel expands its settlements, which are now home to more than 500,000 Israelis.

Netanyahu has rejected calls to halt settlement construction, saying that a partial freeze he imposed in 2009 and 2010 failed to restart substantive negotiations. He says talks should resume without any preconditions.

Israeli officials have brushed off the international criticism as either unfair or by portraying it as a disagreement among friends. But officials say the increasingly frosty relations with Europe are a cause for concern.

On Tuesday, the various major sports leagues gathered in New Jersey for a hearing in connection with the attack on the state?s intention to legalize gambling on individual professional games.? The case, which already has generated testimony from the Commissioners of the NFL, MLB, NBA, and NHL, initially turns on an important legal question regarding the ability of a party to file a lawsuit.

It?s called ?standing.?? And without it, a lawsuit can?t be pursued.

New Jersey argues that the sports leagues will suffer no injury, and thus they can?t sue.? ?There?s no evidence that gambling has hurt the sports leagues and their reputation,? said New Jersey?s lawyer (and former U.S. solicitor general) Theodore Olson, via the Associated Press. ?All evidence shows sports have grown in this country alongside the growth of gambling, some legal, some not legal. Illegal gambling on the Super Bowl has made the Super Bowl the most-watched event on TV in the world.?

Jeffrey Mishkin, who represents the sports leagues, obviously disagrees.? ?There will be greater suspicion about all of the normal incidents in the game, every dropped pass, every missed free throw,? Mishkin said.? ?They?re our games.? That gives us a personal stake, and that gives us standing.?

When the sports leagues successfully shut down Delaware?s effort to launch single-game betting under the same federal law being used against New Jersey, Delaware didn?t argue that the sports leagues have no right to sue.? It?s smart for New Jersey to at least try.? Apart from being a no-lose proposition, there?s no way Jersey can win without doing something different than Delaware did.? After all, the two states reside in the Third U.S. Judicial Circuit, which means that the ultimate outcome on appeal from the Delaware case (which Delaware lost) will be binding on New Jersey if the same arguments are made.

New Jersey also is expected to attack the constitutionality of the 1992 federal law that restricts efforts by the various states to legalize gambling, another argument on which Delaware deferred.

A ruling on the ?standing? issue is expected by Friday.? Until then, perhaps New Jersey can take bets on the outcome.

Iraqi president in hospital after suffering stroke

BAGHDAD (Reuters) - Iraqi President Jalal Talabani, a Kurd who has mediated among Shi'ite, Sunni and Kurdish parties, was in hospital on Tuesday after suffering a stroke that left him in "critical but stable condition", government officials and lawmakers said.

Without Talabani, Iraq would lose an influential peace-maker who often eased tensions in the fragile power-sharing government and negotiated in the growing rift over oil between Baghdad and the OPEC member country's autonomous Kurdistan region.

Reports on his medical condition varied. Three government sources said he was in critical condition, but his office said the 79-year-old president was stable under intensive medical supervision after receiving treatment for blocked arteries.

"President Talabani has suffered a light stroke. His condition is stable now and doctors are closely monitoring him and if they decide he should be transferred outside then he'll go," veteran Kurdish politician Mahmoud Othman, a close Talabani associate who was in the Baghdad hospital.

Talabani had been suffering from ill health much of this year and received medical treatment overseas several times in the last two years.

Prime Minister Nuri al-Maliki visited the hospital earlier on Tuesday.

TOUGH TIMES AHEAD?

Under Iraq's constitution, the parliament should elect a new president if the post becomes vacant and Iraq's power-sharing deal calls for the presidency to go to a Kurd while two vice presidents are shared by a Sunni Muslim and a Shi'ite Muslim.

Political analysts said former Kurdistan prime minister Barham Salih is favored candidate to replace Talabani should the president be incapacitated.

But his exit from Iraqi politics would come at a sensitive time and any succession would be complicated, a year after the last American troops left the country.

"He is the most moderate among Iraqi politicians and the most able to defuse political shocks. I do not think any one will be able to fill his position as a president and as a politician," Iraqi analyst Ibrahim al-Sumaidaie said.

Iraq law would see one of the vice presidents take over Talabani's duties before the parliamentary vote. But Iraq's Sunni Vice President, Tareq al-Hashemi, is a fugitive outside of the country after he fled to escape charges he ran death squads. He was sentenced to death in absentia.

Any parliament vote would also be complex, with Maliki locked in a struggle with Sunni, Kurdish and some Shi'ite rivals in the power-sharing government. Talabani was crucial in helping the Shi'ite leader survive a no-confidence motion directed against him earlier this year.

Talabani also recently helped ease a military stand-off between Maliki's central government and the autonomous Kurdistan president, Masoud Barzani, in their long-running dispute over oil-field rights and internal boundaries.

But that situation remains sensitive after both regions sent troops to reinforce positions along their internal frontier.

A veteran of the Kurdish guerrilla movement, Talabani survived wars, exile and infighting in northern Iraq to become the country's first Kurdish president a few years after the 2003 invasion that toppled Saddam Hussein.

(AP) ? The pregnant Duchess of Cambridge made her first public appearance Sunday night since her hospitalization for acute morning sickness.

Looking healthy and strong, the former Kate Middleton presented awards at the BBC Sports Personality of the Year prizes, which were broadcast throughout Britain.

She wore a dark green outfit and seemed composed and comfortable, showing no signs of the acute morning sickness that had led to her recent hospitalization.

She had not been seen in public since leaving King Edward VII Hospital in London on Dec. 6. At the time, officials said the duchess would be resting.

At the conclusion of the gala show, she presented the lifetime achievement award to Sebastian Coe, the newly elected chairman of the British Olympic Association and a two-time Olympic middle-distance champion. She also presented the sports personality of the year award, won by Tour de France and Olympic champion Bradley Wiggins.

Palace officials have said the Duchess is in the first 12 weeks of her pregnancy. It would be the first child for her and her husband, Prince William.

It was also her first public appearance since the apparent suicide of a nurse at King Edward VII Hospital who was the victim of a prank call by two DJs from an Australian radio show.

Cox Communications reports email outages in RI | Turn to 10

Cox Communications said Monday that it has resolved the email outage that affected hundreds of residential customers across Rhode Island.

Cox Communications said the outage affected several states, including Rhode Island. Business services were not effected, it said.

Cox said no messages will be lost and that it has no indication of a security breach.

The company released a statement Monday morning:

As of 6:30 am ET, access to Cox email has been restored to all customers previously affected by the email outage. All customers should now be able to send and receive email messages.?

If you lost access to your email during this outage, we have queued your emails received since Friday. You may continue to receive these queued emails over the course of the next several days. These will arrive gradually and may not be delivered in chronological order.?

Now that service is restored we are moving forward with replacing email storage platform equipment and implementing measures to prevent a reoccurrence of these issues. We will remain intensely focused on this effort until all queued email messages are successfully delivered. Technical teams continue to be on high alert and monitoring systems closely.?

We deeply regret the impact this outage has had on our customers and truly appreciate their patience as all Cox resources continue to be focused on this restoration effort.?

AG-184C - Government owned. Records indicate the seal has not yet been broken, however there are indications of malfunction and this can be incorrect. Communication was strong with this bunker until twenty years ago. At time of last transmittal, the survivors were made up of military personnel, government agencies and local families. Records were being studiously kept and genealogy was tracked. They preferred to keep lines of communication professional, but the following information is known.

There were several mentally ill survivors that often had to be restrained, some deaths occurred before they were terminated. The population was not comfortable, with several fights reported. a surge in births occurred every ten years, followed by a sharp decline in older survivors. Communication lines were only opened with men, females were never heard in the background. As a result of a large scaled riot, there was a change in 'management' shortly before communication was ceased.

AG-185C - Government owned. Records indicate the seal has sustained damage from both inside and outside, it is unclear if it has been broken. There was no communication with this bunker since the event. Survivors are unknown, status is unknown.

AG-186C - Government owned. Records indicate the seal was recently broken from the inside. Communication with this bunker has provided the following information. This bunker was filled primarily with doctors, teachers, and Navy personnel from a nearby base. There were also several wealthy families that managed to bully their way into the bunker through monetary bribes. Fights break out often in this bunker, for the descendants, of the wealthy families in particular, still seem to believe they have a sort of privilege over the others. They are reported as being rude and strongly disliked by the other AG-186C residents. The atmosphere pretty much came to a boil recently. It is predicted that when those doors open, the groups will be pouring out and scattering to the wind.

The doctors within AG-186C have kept close tabs on the children and genealogy of the bunker residents. With the help of the teachers, special care was taken to keep records of couples and children that have been born during their time underground so that bloodlines are kept clean and pure. The doctors and teachers have met with some of the higher ranking Naval officers twice a week for planning for procreation. Women have been regarded very highly in this bunker; and some of them are reported to have taken full advantage of that, walking around as if they own the place and treating others like dirt. Of course, many women registered protests against the decisions that had been made to use their bodies for child bearing, but many others have gladly accepted the calling.

CPB-275 - Independently owned. Unable to track the seal's status on independently owned bunkers. This bunker is registered as a medium class bunker. Communication ended fifty years ago. Extremely hostile, number of survivors unknown. They did not want to reveal information and grew angrier when questioned further. General turmoil was a constant background noise, including gunshots and screaming. Recent status unknown.
